- MESH staff worked with both the RSPCA and EDF energy to review existing
risk assessments to determine if human error had been appropriately
considered.
- MESH worked with a large paper company in Kent to review its risk
assessments covering one of its paper machines. The aim was to recognise
the implications of human error when assessing the risks. The types
of issues that were quickly identified that had previously not been
considered were:
- The effects of custom and practice
- The working methods applied by different shifts
- Incorrect tooling being used
- A lack of training covering situations that were not the norm
This feedback was relayed to the relevant managers in order for them to review the assessments covering other production areas.
