About Accident Investigation
Accidents are a learning opportunity because today they may not cost you anything but if lessons aren't learnt from accidents then the impacts later are high and could involve injury, death, legal action, or even imprisonment.
Reporting accidents and ill health at work is a legal requirement. If you are an employer, self employed or in control of a premises you will have duties under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR).
Accidents that result in the following will need to be reported to your enforcing authority:
Death and major injury
Must be reported without delay and must be followed up with a completed accident report (F2508) within 10 days (includes fractures, amputations, loss of sight, injuries requiring admittance to hospital of more than 24 hours etc).
Over 3-day injury
A completed accident report form (F2508) must be sent within 10 days.
Disease
If notified by a doctor that your employee suffers from a reportable work-related disease (including diseases such as carpal tunnel syndrome, occupational dermatitis, occupational asthma etc.), you must send a completed F2508A report form.
Dangerous occurrence
If something happens that could have resulted in a reportable injury but didn't (such as failure of any closed pressure vessel, collapse of scaffolding or building, explosion or fire etc) this may need to be reported immediately.

MESH are often requested to assist clients in the investigation of accidents when a RIDDOR report is needed to be made. This occurred at a Kent based sign company in 2008 when an accident in their yard occurred involving a fork lift truck and an employee. This resulted in the employee sustained a back injury requiring a prolonged absence from work.
We assisted the company in leading the investigation process and the completion of the RIDDOR report. This incident was subsequently followed up by the HSE but due to the improvement actions already undertaken they were happy to accept that the necessary improvements had been undertaken and took no further action.
